At General Practice Panacea, several POHs work in support of the general practitioners.
A POH (GP Practice Support) provides delegated, medical-related general practitioner care, in particular to patients with a chronic illness. For this the POH has more time than the general practitioner and works in close consultation with the general practitioner.
There is a POH-S consultation hour and a POH-GGZ consultation hour.

You can be referred to this consultation hour for:- diabetes mellitus
Patients with diabetes mellitus can go here for checks, advice and explanation. - cardio vascular diseases (CVRM)
CVRM stands for cardio vascular risk management. In the CVRM care program the POH-S calculates on the basis of blood pressure, cholesterol, smoking or not, gender and age, which is the risk of getting cardio vascular diseases. In the event of an increased risk, the patient decides together with the POH-S with what measures the risk can best be reduced. - asthma/COPD
Patients with asthma or COPD can go here for checks, advice and explanation. - geriatrics
Vulnerable elderly people may more often have health problems and concerns that are less well recognized. Patients who belong to this group can go here for care that is appropriate in their situation.

You can be referred to this consultation hour for:- psychological complaints
- psychosocial complaints
To a POH consultation hour you have to be referred by the general practitioner. Consultations take place according to appointment.
