Landelijk Schakelpunt (LSP - or National Exchange Point)

Our GP practice is connected to the Landelijk Schakelpunt (LSP - or National Exchange Point).

The LSP makes it possible for healthcare providers to exchange information about your health via the internet via a secure connection. 
The purpose of this is, in relevant situations, to be as well informed about your health as possible so the most adequate care can be provided to you.

Via the LSP information can be exchanged between your general practitioner, your pharmacy, the GP post and your treating medical specialists. This concerns a part of the data from your GP file and your medication file.

The GP post
If you go to the GP post, for a situation that can not wait, when the GP practice is closed, the GP post has access to a part of the data that the GP practice administers.
The GP post in turn shares an observation message with the GP practice. This way the general practitioner knows what complaints you have been at the GP post and what has been done in response to this.

Your pharmacy
Your pharmacy has, in relevant situations, access to your medication data. This involves the medication the general practitioner has prescribed to you, but also any intolerances, contraindications and allergies (ICA data). They can then take this into account when providing medication.

Your treating medical specialists
Your treating medical specialists have, in relevant situations, access to your medication data. This involves the medication the general practitioner has prescribed to you, but also any intolerances, contraindications and allergies (ICA data). They can then take this into account when prescribing and providing medication.

You must give your consent for the exchange of your data via the LSP.
If you want us to share information about you via the LSP, you can arrange your consent in the following ways:

  • In person
    You can tell your general practitioner and pharmacy.
     
  • Written
    You can download, print and complete this permission form and give it to your general practitioner and pharmacy.
     
  • Online
    You can give your consent via the website Volg je zorg.
